I can't remember a party wipe that was solely my fault, but I have definitely gone into RoF with portions of my necromancer's armor not infused, with predictable results.

in a FOW B/P party, as we entered, we decided to aggro the group on the left and to not touch the one on the right before the left one is done. I, being all adrenalined, hit space bar and barraging at the wrong group... yea, we beat some records at fast whiping...
well...... they made me pay for the next run

FOW group, our mes thought he could tank because he had DISTORTION :0
after he died 3 times, he got like 3 shards in a row. He died again. I decided that this guy didnt deserve crap for drops because he complained about inadequate healing. So... I aggroed 5 mobs and said "LOOKIT MEEE I CAN TANK LOLZ" I got some angry PMs for the next hour. I didnt bother no ignore him because i found it so ammusing.

I cannot tell you how many times I have done something similar... However I usually manage to pull out before the damage gets done... and I think me and my bro have you beat on time... We just went down there cause we had never been (just me and him being newbs) and got killed so fast!

I was doing Aurora Glade with my E/Mo late one night. Teamed up with a Warrior and a Ranger, rest of the group was hench. Being the weekend I'd had a few glasses of the old vino thinking "Ah, this misshhh is a walk 'n the park, eashy peashy *hic*... *more wine*". So we proceed throught the mission with no problem, me merrily yattering away to them and finishing off my bottle of wine, when we start approaching the final area. I'm saying "just get the crystals in place, forget the white mantle, blah blah."
War - "F*** that, I'm killing everything!"
Me - "Stop! Wait a minute"
War - "WTF?"
Me - "Don't attack them, we'll fail" (with much drunken typoness)
War - "What then?"
Me - "Well we have to place the crystals... blah blah"
...
...
*Mission failed. Your party was defeated etc etc.*
...
...
Me - "Oops..."
As you can guess, this was followed by much profanity and name-calling from the others. The ranger continued to rant at me in whispers to which I responded in pretty much the same silly ranting manner. I decided that was going nowhere, promptly apologised to him and wished him luck as he was doing the mission again. He came back and helped my ele through the mission and I ended up talking to him into the early hours of the morning.
Sometimes it's nice to just own up and say "yeah, my fault, sorry" instead of trying to argue the point which I initially did.
Oh, I also learned it's not a good idea to drink and play Guild Wars

Oh, and Tryll? I played with a really drunk Monk once. I was starting to wonder what I was doing wrong when she told me. She said her husband insisted I know so I didn't think she sucked. hehe

lol, that actually happened to me just the other day. Got a new set of Factions armor, didn't even think of getting it infused (forgot all about infusion it's been so long). Went back to help on a fire isle mission (forget which one). The second after a Jade hit me, I remembered that fun word "infusion". We always use vent to chat, and the roar of laughter in my ears was defeaning.
The best wipes are those where someone, anyone says "Ooops" just seconds before a small river of red dots flows over the group.
What makes it more fun is when no one screams and yells about it. It's when everyone gets a laugh and keeps going or tries again. Good way to build friends and have fun.

OMG dude, OOPS happens in our guild so often -_- Rarely do we take it badly, usually we just deal or if it was so amusing it couldnt be anything else - we LAUGH! And I have met a few pugs that can just laugh it up. Great people!

There I was, Monking for a group doing Dunes of Despair (I had just hit level 20 at the time). Before we started, I had to redo my skill bar from Smiting (my Henchie build) to Healing (my PUG build). I grabbed rebirth instead of ressurect. (Can you see where this is going?) So we get to the pedestal and get the Hero up there, and then the trouble started. One guy gets spiked and dies, too bad he was the only one carrying a rez sig. I hit rebirth, lost my energy, and that was that.
I'm a MUCH better Monk now!!!
